Start date
– the date

Terminal
– the end stages of a
medical condition
where life expectancy is
considered to be days or weeks and only
palliative
treatment
is given.
Therapist
– an osteopath, chiropractor, homeopath, podiatrist, acupuncturist or
Chinese herbalist who is qualified and licensed in the country where
treatment
is
provided.
Treatment
– any surgical or medical services, including
diagnostic tests and
procedures
